Who We Serve

We provide specialized residential care for two distinct but overlapping populations.

Adults with Intellectual Disabilities (IDD)

We provide residential support for adults with intellectual disabilities who require structured, supervised living with 24/7 DSP support for activities of daily living.

Admission Criteria

  • Diagnosed with an intellectual disability
  • Age 18 or older
  • Enrolled in or eligible for a DD waiver
  • Referred by a CSB support coordinator
  • Requires 24/7 residential support

Adults with Dual Diagnosis (IDD + Mental Health)

We specialize in serving adults who have both an intellectual disability and a co-occurring mental health condition. Our staff are trained to support the complex needs of this population.

Admission Criteria

  • Primary diagnosis of IDD
  • Co-occurring mental health diagnosis
  • Age 18 or older
  • Enrolled in or eligible for a DD waiver
  • Behavioral support plan in place or in development

Important: We do not serve individuals with mental health diagnoses only.

Daily Support

24/7 Activities of Daily Living Support

Our Direct Support Professionals provide comprehensive assistance with all activities of daily living, ensuring each individual can live as independently as possible while receiving the support they need.

Support is always individualized — we follow each person's ISP and work to build skills, promote autonomy, and respect personal preferences in every interaction.

Group home residents and DSP staff

Personal Hygiene & Grooming

Bathing, dressing, grooming, and oral hygiene assistance tailored to each individual's needs and preferences.

Meals & Nutrition

Nutritious, home-cooked meals planned around individual dietary needs, preferences, and any medical restrictions including modified textures and thickened liquids.

Medication Administration

Trained staff administer all medications per MAR and physician orders, maintaining accurate logs and monitoring for side effects and adverse reactions.

Safety Monitoring

24/7 supervision and safety monitoring to prevent accidents, respond to emergencies, and ensure a secure home environment.

Health Coordination

Coordination with physicians, specialists, and CSB health services for appointments, follow-ups, and health monitoring.

Community Integration

Support for community outings, recreational activities, and social engagement to promote independence and quality of life.

High-Acuity Specialty

Heavy Medical Supports

We handle high-risk medical profiles that many residential providers cannot safely support. Our DSPs are trained, supervised, and regularly evaluated on medical and safety competencies to support high-acuity individuals — managing day-to-day medical needs within the home while coordinating with nursing professionals, physicians, and home health providers for oversight and specialized care.

Respiratory & Breathing Support

  • Oxygen administration (continuous or PRN)
  • Nebulizer treatments

  • Monitoring for respiratory distress or decline

Aspiration & Feeding Support

  • Aspiration precautions (positioning, pacing, supervision)
  • Modified diets (puree, mechanical soft, thickened liquids)
  • Full and partial feeding assistance
  • G-tube / enteral feeding support
  • Monitoring for choking, coughing, and pocketing

Medication Management (High Oversight)

  • Full medication administration per MAR
  • Crushed medications and altered administration
  • PRN medication tracking and effectiveness logs
  • Monitoring for side effects and adverse reactions
  • Medication reconciliation and pharmacy coordination

Seizure Management

  • Monitoring for seizure activity and triggers
  • Seizure protocols (timing, positioning, safety)
  • Emergency response and 911 coordination
  • Post-seizure care and documentation
  • Medication compliance for seizure control

Diabetes & Blood Sugar Management

  • Blood glucose monitoring
  • Insulin administration
  • Monitoring for hypo/hyperglycemia symptoms
  • Diet coordination with medical needs
  • Physician and endocrinology coordination

Skin Integrity & Pressure Injury Prevention

  • Repositioning schedules
  • Skin checks and detailed documentation
  • Barrier creams and wound care support
  • Monitoring for redness, breakdown, and infection
  • Use of pressure-relief equipment

Bowel & Bladder (High-Risk Conditions)

  • Chronic constipation management and bowel protocols
  • Monitoring for bowel obstruction
  • Incontinence care (brief changes, hygiene)
  • UTI monitoring and prevention
  • Scheduled intervention documentation

Mobility, Transfers & Physical Support

  • Hoyer lift use and safe transfer techniques
  • Full assist transfers
  • Gait belt support
  • Fall prevention interventions
  • Wheelchair positioning and safety

Chronic Condition Monitoring

  • Hypertension monitoring (blood pressure checks)
  • Edema monitoring (leg swelling, wrapping)
  • Heart-related symptom awareness
  • General health decline monitoring
  • Hydration and dehydration prevention

Infection Prevention & Control

  • Monitoring for signs of infection (fever, behavior changes)
  • Sepsis awareness and rapid response
  • PPE and hygiene protocols
  • Coordination with medical providers
  • Isolation and containment procedures

Nighttime Medical Monitoring

  • Overnight safety checks
  • Positioning and comfort adjustments
  • Monitoring for breathing issues and seizures
  • CPAP / oxygen oversight overnight
  • Incontinence care during nighttime hours

Behavioral + Medical Overlap

  • Self-injury tied to medical discomfort
  • Emotional outbursts related to pain or health changes
  • Medication-related behavior monitoring
  • Redirection tied to health and safety needs
  • Coordination with behavioral support teams

Our Medical Staffing Model

Our DSPs are trained and competency-validated to support individuals with complex medical needs, including medication administration, aspiration precautions, mobility support, and chronic condition monitoring. We coordinate closely with licensed nurses and external healthcare providers for clinical oversight, specialized treatments, and any needs requiring skilled nursing services.

We utilize nursing oversight and coordinate with home health when clinically indicated, but our DSP team is fully trained to manage the daily medical needs of the individuals we support — preventing hospitalizations and managing chronic and acute conditions with consistent documentation.

End-of-Life Care Partnership

Hospice & Palliative Care, In Place

Soul Spring works hand in hand with Hospice and Palliative Care teams so that residents never have to leave the home they know when their health declines. Our staff support comfort-focused care protocols alongside visiting Hospice nurses — providing continuity, dignity, and peace for residents and families through every stage of life.
