We've designed our referral process to minimize friction for case managers and CSBs. From submission to placement, we keep you informed at every step.
Submit the Referral Form
Complete our secure online referral form with all required client information. The form captures client demographics, diagnosis, level of care, CSB details, urgency, and bed preference.
Instant Automated Confirmation
Immediately after submission, you receive an automated confirmation email with your referral tracking number and a summary of next steps. Our system also logs your referral for immediate review.
Clinical Review & Assessment
Our clinical team reviews the referral within 24–48 business hours. We assess fit based on diagnosis, level of care, behavioral needs, and current bed availability. We contact you to discuss and schedule a pre-admission assessment if appropriate.
Placement & Transition Planning
Once approved, we coordinate a comprehensive transition plan with you, the individual, their family or guardian, and the CSB. We ensure a smooth, person-centered move-in experience.

QHow quickly do you respond to referrals?
We respond to all referrals within 24–48 business hours. Urgent or crisis referrals are prioritized and may receive a same-day response.
QDo you have beds available right now?
Yes — we currently have 2 beds available for immediate placement. We also maintain a waitlist for future openings.
QWho can submit a referral?
Referrals must be submitted by a CSB Support Coordinator, CSB-contracted case manager, or authorized referral facilitator. Families may contact us directly, but the formal referral must come through the CSB.
QWhat waivers do you accept?
We accept Community Living, Family and Individual Support, and Building Independence waivers. If you're unsure about waiver eligibility, contact us and we'll help assess options.
QDo you serve individuals with mental health diagnoses only?
No. We specialize in IDD and dual diagnosis (IDD + mental health). We do not serve individuals with mental health diagnoses only.
QWhat happens if there are no immediate beds available?
We place the individual on our waitlist and notify you as soon as a bed becomes available. We keep you updated regularly on waitlist status.
